How to notify us of conflicts of interest and changes to them

Refer to the definition of a conflict of interest in this policy to ensure you notify DCJ of conflicts of interest we need to know about.

You’re not required to notify us of conflicts of interest that are unrelated to your contract with DCJ.

Instructions

Complete the conflict of interest notification form, making full disclosure of all relevant information:

  • immediately when your organisation or any of its personnel identify or become aware of a conflict of interest, or
  • immediately when there’s a change of circumstances to a conflict of interest you’ve previously notified us of.

Provide enough information for us to review and assess the conflict of interest. For example, provide enough detail about the proposed management strategy to ensure it’s feasible, addresses the conflict of interest, and you’d be able to provide evidence of it being managed.

You must complete a separate form for each conflict of interest.

Send the completed form to your DCJ contract manager.
