Change of details

• Change of Details factsheet for children in care (PDF, 417.7 KB) This factsheet explains to caseworkers what is considered to be a change in circumstance or details for a child or young person in care
• Change of Details form for children in care (DOCX, 81.1 KB) This form should be completed by caseworkers to advise the OOHC Health Coordinator and NSW Education each time there is a change in a child or young person’s circumstances or details within 7 days.
